If you ask the internet, Botox lasts “three to four months.” If you ask me — after twenty-plus years in healthcare and eleven in aesthetics — the honest answer is: it depends, and your first round is usually the shortest.
Here’s the version I give my patients in the chair.
The typical window
For most people, Botox results settle in over the first two weeks and hold for roughly three to four months. Around month three, you’ll usually notice movement creeping back — a little more lift in the brows, a faint line where things were smooth. That’s not the product “failing.” That’s your body doing exactly what it’s supposed to do: slowly rebuilding the nerve signal to the muscle.
Why your first round may wear off faster
This is the part nobody warns you about. First-timers often metabolize their first treatment a bit faster than later ones, for a few reasons:
- Strong, untrained muscles. If your forehead has been expressive for thirty years, those muscles are conditioned. The first round is working against peak strength.
- Conservative dosing — on purpose. When I treat someone new, I’d rather start lower and adjust at a follow-up than overshoot and leave you frozen for three months. A conservative first dose can mean a shorter first result. I consider that a fair trade, and most patients agree once they see the natural result.
- Expectations. You notice every millimeter of returning movement when it’s new. By round three, you know your pattern.
With consistent treatment, many patients find their results stretch longer — the muscles adapt, and maintenance appointments get easier to predict.
What changes the math
A few things reliably influence how long your results hold:
- Metabolism and activity. Very active patients — endurance athletes especially — often report shorter duration.
- Treatment area. Crow’s feet and forehead lines move constantly; areas that work harder tend to soften sooner.
- Dose relative to muscle strength. This is a planning conversation, not a menu order — it’s exactly what your consult is for.
The maintenance rhythm
Most of my regulars settle into a three-to-four-month rhythm. Some stretch to five. The goal isn’t to chase a frozen face — it’s to book your next visit just before full movement returns, so the muscles never get back to full training strength. That’s how results start lasting longer over time.

The part that’s on the label: Botox Cosmetic is FDA-approved for the temporary improvement of frown lines, crow’s feet and forehead lines — the word “temporary” is Allergan’s, not mine — which is why results fade and additional treatment is needed to keep them. Our Botox page lists three to six months. Ninety days is the front of that window, and it’s where the first “I think it’s wearing off” text usually lands in my Chandler treatment room. That’s the body re-establishing the nerve signal, exactly as designed — not the product failing.
Does the dose change how long Botox lasts?
